Keep test ranges for dynamic test up-to-date using Outline info

Currently we update the range of tests as you edit the document only if they are Outline tests. If there are child dynamic nodes, they are left to become stale. There's no reason we shouldn't be able to update them too (eg. when updating the range of a node, also update any childrens ranges if they are Results and have the same range).
