Respect an already pushed branch in pr create #681

  1. If the branch has been already published to a GitHub remote, avoid ever auto-pushing it to a different remote during gh pr create.

  2. When the branch is detected to be already published and its remote tracking branch matches the tip of the local branch, avoid doing git push during gh pr create as the operation is unnecessary.
