Root cause

A terminal ACP process failure left the persisted transcript intact, but resume paths still attempted runtime attachment. Prompt admission and explicit attach could reach ACP session/load, turning a recoverable dead runtime into an internal error. Repeated transcript reads also risked coupling history access to runtime recovery.

Solution

Centralized dead-runtime admission in the shared resume/attach owner, before ACP startup or session/load.
Kept persisted transcript projection independent from runtime attachment; dead sessions remain readable.
Returned the existing not-attachable conflict contract for HTTP and UDS prompt/attach operations.
Preserved unbound logical resumes, which do not require an ACP attachment.
Made the session UI truthful: durable history remains visible, sending is disabled, and users can fork a new child session.
Added the missing default-agent configuration validation baseline from fc11be1f as cd342a33.

The final local make gate-full was intentionally not rerun after the corrected baseline, per execution direction; CI owns the full gate for this PR.
