| web/src/systems/os/hooks/use-workspaces-switcher.ts | Implements the switcher's focus and keyboard model, but persistent numeric indices become stale when live workspace or worktree lists change. |
| web/src/systems/os/components/os-workspaces-overview.tsx | Orchestrates the redesigned overlay and live menu projection; its row focus and activation inherit the stale-index defect. |
| internal/store/globaldb/schema/migrations/00063_schema.sql | Replaces full name uniqueness with a matching partial index so dismissed tombstones release names while retaining history. |
| internal/worktree/exit_actions.go | Resolves exit operations to canonical worktree IDs before applying controls and events. |
| internal/cli/worktree.go | Preserves canonical worktree identity in mutation output through pre-resolution and structured result bundles. |

Stale indices misroute activation
When a removal or catalog update changes the worktree list while the overview remains open, menuIndex still addresses the old array position, causing the focus plate and Enter activation to move to another worktree or disappear entirely. The strip has the same problem when workspace polling changes entries without reconciling focusIndex.

| expected: ⇧⌘W toggles a full-screen overlay over the inert live shell — a single-row glass strip of monogram tiles that never wraps, with edge fades and wheel/drag scrubbing on overflow. Arrows move a frosted focus plate with modulo wrap; Home/End jump; typeahead prefix-jumps with no visible UI; the caption crossfades to the focused workspace's name and ~-contracted root_dir. The current workspace carries a success check on its well — a git-branch glyph while a worktree is scoped, when the check moves to the scoped menu row instead. ↵ or click on a tile always switches to the workspace root and closes; while Global is on no tile reads current, the notice line renders, and picking a tile turns Global off in one gesture. The focused git-backed workspace always shows its worktrees as a vertical menu (locked sort, adopted-only counts, truncation at five plus a New worktree footer row; lone dashed button at zero; nothing at all for non-git). ↓ enters the menu, ↑ at the top or esc returns to the strip, inert rows are skipped with their reasons visible, ↵ scopes workspace/worktree, and each row's kebab (pointer or Shift+F10) offers Copy path and the gated Delete worktree…. Esc steps popover → menu → overlay, returning focus to the menubar chip. Zero project workspaces collapse the strip to the empty copy with the surface's only accent button. | ||
| entry_points: ⇧⌘W; Go menu → Workspaces…; S1 Workspace menu → Workspaces overview… | ||

There was a problem hiding this comment.
This new user-visible scenario and the reset reconciliation and create/adopt scenarios remain untested at workstream completion. The repository requires each affected scenario to carry a completed verdict or an allowed blocked status, so the redesigned and moved interaction paths still lack the required end-to-end QA record.

existing “Could not copy the path” error toast and return, while preserving the
current success and rejected-promise toast behavior.
- Around line 138-149: Update the worktree row’s accessible naming around the
option element and its `aria-label` so assistive technologies receive the row
name plus the visible state, path, and `entry.inertReason`; use an accessible
description referencing the existing state/reason nodes if that better preserves
their text, while ensuring the functional reason is announced rather than
hover-only.

- Around line 196-243: Update onStageKeyDown to return immediately when
event.target is inside an element marked data-slot="dropdown-menu-trigger" or
data-slot="dropdown-menu-content", before stage key handling runs. Add tests
covering Enter and Space key events on the kebab trigger to ensure the worktree
actions menu retains control.
In `@web/src/systems/workspace/hooks/use-worktree-create-dialog.ts`:
- Around line 204-233: Guard the create-dialog submission flow so a second
worktree cannot be submitted while pendingWorktree exists and creationError is

after the pending worktree reaches a terminal success or failure outcome, and
add a test covering a second submit before the first worktree completes.

Guard the clipboard call.
navigator.clipboard is undefined in insecure contexts. navigator.clipboard.writeText(path) then throws a TypeError synchronously, before any promise exists. The error toast never renders, and the exception is unhandled.

Replace the 264px dossier grid in os-workspaces-overview with the macOS Command-Tab overlay locked in docs/design/opendesign/workspaces: a single-row glass strip of 86px monogram tiles over the inert live shell, a frosted plate as the sole focus indicator, an identity caption, and the focused git-backed workspace's worktrees as an always-visible vertical menu with kebab actions. - New os composites (strip, tile, caption, worktree menu/row, hints) plus the roving-focus/typeahead switcher and scroll/drag/snap hooks; menu model in workspaces-overview-model. - Register workspaces.overview (shift+cmd+W) in the window-manager command registry; drop the per-workspace details fan-out (subtitle counts only) and delete use-workspace-details. - Shell-tier glass/plate/veil tokens in tokens.css with DESIGN.md regenerated; delete the dead workspace-card tokens. - Display-contract home-rooted paths through contractHomePath/WorktreePath so captions and menu rows read ~-contracted while clipboard and tooltips keep absolute paths. - Rewrite the stories to the state-matrix lab (12 incl. row-actions-open), add the owning 17-test interaction suite, update os-shell/worktrees e2e to the vertical-menu interaction, and add RT-workspace-overview-command-tab to the QA tracker. 🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win
Assert the wrapper prefix as well as the cause.
errors.Is(err, storeErr) passes for both the required wrapped error and a raw storeErr. The Resolve contract adds the worktree: resolve registry row context. Add a message assertion so this test fails if that context is removed.
Proposed assertion
- if !errors.Is(err, storeErr) || errors.Is(err, ErrNotFound) || item != nil {
+ if !errors.Is(err, storeErr) ||
+ !strings.HasPrefix(err.Error(), "worktree: resolve registry row:") ||
+ errors.Is(err, ErrNotFound) ||
+ item != nil {As per path instructions, Go tests must use specific error assertions such as ErrorContains or ErrorAs.

The old 264px dossier grid is hard-cut per Zero Legacy Tolerance.c516a65) — the in-flight follow-ups to feat: add first-class worktree support #388: dismissed tombstones free their reserved name, exit actions resolve caller refs to canonical record ids, and CLI mutation output keeps worktree identity.1. Workspaces overview — Command-Tab switcher
Why
Production
os-workspaces-overview.tsxwas a full-screen dialog that read as a dossier grid: 264px cards with member stacks, path footers, and an "Enter →" row. None of that matches how people actually switch workspaces: glance, arrow, land. The redesign (DESIGN-NOTES.md, locked decisions) makes the overview a Command-Tab overlay over the live shell — workspace identity switching only; window arrangements stay with the Desktops overview (⇧⌘S).The visual contract
