under narrower policies while preserving the existing mode checks.
In `@internal/config/provider_test.go`:
- Around line 1136-1155: Use the required t.Run("Should...") pattern for all
three test cases: in internal/config/provider_test.go lines 1136-1155, move
TestWorkspaceCannotEnableProviderFullAccess’s body into a subtest named "Should
reject enabling provider full access from workspace config"; in
internal/acp/types_test.go lines 18-19, prefix each mode subtest name with
"Should require approve-all for unrestricted mode "; and in
internal/session/manager_transition_test.go lines 26-68, move the test body into
a subtest named "Should refuse an unrestricted mode for a persisted restricted
session".
In `@internal/daemon/loop_action_policy.go`:
- Around line 99-102: Update the option handling around
allowedLoopAgentACPOptions and resolvedAgent.ACPOptionsValue to distinguish
authored agent selections from provider-injected fallback modes. Preserve
authored agent-full-access and bypassPermissions selections so the existing
validation in internal/acp/types.go returns the “requires approve-all” error
under approve-reads, while continuing to filter provider-injected fallback
modes.
In `@internal/session/manager_prompt_runtime.go`:
- Around line 292-297: Update the ACP mode handling in Manager.Create and later
runtime changes so applyProviderFullAccessPreference’s injected unrestricted
mode is removed when effective session permissions are narrower than
approve-all. Preserve validation failures for unrestricted modes explicitly
selected by the user or agent.

Codex review via the authenticated pedronauck account: inspected the final ACP permission guard, global preference and scope, session and Loop runtime precedence, regression coverage, merged mainline compatibility, and targeted live QA evidence. Local merged-tree Go lint and race gate passed. Approval covers that reviewed code and evidence; merge remains contingent on all applicable exact-head CI lanes and Vercel succeeding. This does not represent a separate human review.
What & why
Add an explicit global
permissions.provider_full_accesspreference for trusted ACP sessions. Withapprove-all, it selects Codexagent-full-accessor ClaudebypassPermissions; other providers fail clearly. Explicit agent ACP modes still take precedence. Unrestricted modes are rejected when the effective session permissions are narrower, including during live runtime changes.
