Closes #689. Related to #678 only where it shares SQLite writer pressure.

Problem and root cause

An idle Loop reconciliation cycle took the SQLite write lock before discovering whether there
was any work. Its correlated orphan query repeatedly searched historical task runs without a
general Loop ownership index. Provenance backfill also scanned all coordinator history under the
writer on every cycle. A cycle reporting zero examined runs could therefore block action-result
and session writes for seconds, with increasing cost as history grew.

After a tool returned, its result/event write could expire. The action runtime then dropped the
result or persisted a transport failure, allowing side effects to execute again. Lease heartbeat
contention could also cancel a healthy tool. The automation scheduler retried a failed claim with
zero delay because its scheduled fire remained overdue.

Changes

  • Discover orphan candidates through indexed live/unsettled records without taking the writer.
Recheck the Loop's terminal status and settle each candidate in a separate transaction.
  • Run provenance repair after readiness until successful, read outside the writer, and apply
conditional per-task updates without replacing concurrent metadata changes. Select the latest
queued coordinator once per task, using its run ID to break timestamp ties.
  • Add lossless global migration 00126 for the ownership/provenance/unsettled indexes.
  • Skip write admission for heartbeat retention when no rows have expired.
  • Retain action outcomes and retry only completion/failure, heartbeat and tool-event persistence
    with fresh bounded attempts and cancellable backoff. Do not invoke the tool or hooks again;
preserve existing permanent validation failures and committed/publication error handling.
Reserve pending settlement ownership against automatic lease recovery, keep the original command
timestamp across attempts, and allow bounded shutdown grace for a completed result.
  • Back off failed scheduled claims exponentially from one second to thirty seconds while retaining
    the original scheduled fire identity.

Verification

  • Reproduced the original failure against real SQLite with 12,000 settled task runs and a held
writer; the corrected empty sweep and provenance pass without acquiring that writer.
  • Real task/Loop integration covers completion and failure writes spanning multiple attempt
deadlines, contention during heartbeat, one tool invocation, terminal generation output and a
released lease. Completed tool-event persistence produces one event after unlock.
  • Real scheduler integration with an injected clock verifies spaced retries and one original fire.
  • Migration 125→126 preserves existing task metadata/results across repeated reopen; existing
migration integrity and schema-equivalence suites pass.
  • Additional real-store coverage verifies shutdown during persistence, retries past the lease
deadline, recovery isolation and reservation release, and deterministic provenance backfill.
  • The existing managed ACP skill/session contention integration also passes.
  • All focused checks use CGO_ENABLED=1 and -race. make codegen and make codegen-check pass.
  • make gate passes all affected lanes: codegen check, Go lint with zero issues, and the complete
automation, daemon, store and task race suites. The test-convention check has no new findings.
Further delivery validation uses CI without repeating heavy local suites.

Cross-surface impact: docs/_memory/change-impact.md, “Issue 689”. No public command, DTO, tool ID,
configuration or official skill syntax changes. QA acceptance and evidence:
docs/qa/reports/2026-10-03-issue-689-sqlite-contention.md.

Summary by CodeRabbit

  • Bug Fixes
    • Scheduled jobs now retry after dispatch failures, with delays that increase gradually and stop when the scheduler is canceled.
    • Task completion, failure, and event records are more resilient to temporary database contention, reducing the chance of lost updates or duplicate execution.
    • Loop-run reconciliation and heartbeat cleanup handle database activity more reliably, including when there is no expired data to remove.
    • Startup provenance repairs can be retried after a failure and are not repeated once completed.

Walkthrough

The change adds bounded retries for scheduled-job dispatch and daemon persistence. Action settlement reserves leases while writing results, and lease recovery excludes reserved runs. Loop reconciliation uses updated queries and transactions. Heartbeat-wake sweeps skip writes when no events have expired.

Changes

Scheduled job dispatch

Layer / File(s) Summary
Dispatch retry backoff
internal/automation/schedule_execution.go, internal/automation/schedule_integration_test.go
Dispatch failures use a logged delay that grows to 30 seconds. Cancellation stops the loop, and successful dispatch resets the delay. The integration test checks that a one-shot fire dispatches once after a held SQLite write lock is released.

Daemon persistence and lease settlement

Layer / File(s) Summary
Retrying daemon persistence
internal/daemon/sqlite_persistence.go, internal/daemon/loop_action_runtime.go, internal/daemon/loop_action_liveness.go, internal/daemon/boot_tool_registry.go, internal/daemon/native_tools.go, internal/daemon/tool_events.go, internal/daemon/*integration*test.go, internal/daemon/task_runtime_test.go, internal/daemon/tool_events_test.go
A shared helper retries selected SQLite persistence errors. Action completion, failure, heartbeat writes, and tool-event writes use retry persistence. Action settlement uses a detached context with a grace period. Timeout classification now requires an execution error. Tests cover write contention, regular and oversized results, shutdown, expiry, and tool-event persistence.
Lease settlement reservation and recovery
internal/task/lease_settlement_reservation.go, internal/store/globaldb/global_db_task_lease_reservations.go, internal/store/globaldb/global_db_task_claim_helpers.go, internal/store/globaldb/global_db_task_claim_lease.go, internal/store/globaldb/queries/task_claim.sql, internal/store/globaldb/sqlcgen/task_claim.sql.go, internal/store/globaldb/repositories.go, internal/task/manager_integration_test.go
The task service and repository reserve a run’s lease settlement. Expired-lease recovery excludes runs with a matching reserved run ID and claim-token hash. Integration tests check recovery before and after reservation release.

Loop reconciliation and heartbeat sweeps

Layer / File(s) Summary
Reconciliation queries and indexed data
internal/store/globaldb/queries/loop_reconciliation.sql, internal/store/globaldb/sqlcgen/loop_reconciliation.sql.go, internal/store/globaldb/schema/definitions/70_tasks.sql, internal/store/globaldb/schema/definitions/72_task_runs.sql, internal/store/globaldb/schema/migrations/00126_schema.sql, internal/store/globaldb/global_db_test.go
New queries list reconciliation candidates and provenance, retrieve loop status, and conditionally update task metadata. The schema and migration add partial indexes for these queries. The migration test checks that task metadata and run result JSON remain unchanged after repeated reopen.
Reconciliation transactions and backfill state
internal/store/globaldb/global_db_loop_run_reconciliation.go, internal/store/globaldb/global_db_loop_test.go, internal/daemon/loop_reconciler_runtime.go, internal/daemon/task_runtime_test.go
Orphan reconciliation settles candidates in separate immediate transactions. Provenance backfill reads and conditionally updates each record in its own transaction. The runtime retries backfill after an unsuccessful attempt. Tests cover contention, backfill selection, readiness, serialized sweeps, and avoiding repeated backfill.
Heartbeat wake sweep pre-check
internal/store/globaldb/queries/heartbeat.sql, internal/store/globaldb/sqlcgen/heartbeat.sql.go, internal/store/globaldb/global_db_heartbeat_wake.go, internal/store/globaldb/global_db_heartbeat_test.go
The sweep checks for expired events before starting its bounded write operation. A test checks an empty sweep while a SQLite writer transaction is held.

Merge Risk: ⚪ Minimal · up to 4cdd2

The claimed lease-reservation race is not reachable. No remaining issue from this review prevents merging after normal checks.

Security Architecture Review

Security architecture risk: 🟡 Moderate · up to 4cdd2

The change improves protection against repeated tool execution during database contention and preserves existing ownership checks. However, repeated persistence failures can keep completed work awaiting settlement indefinitely while the application remains running, preventing normal expired-lease recovery. No privilege escalation or externally reachable attack path was established.

Retained concerns

  • Medium · reliability · inferred: Settlement has bounded attempts but no overall lifetime while the daemon remains active. Repeated retryable persistence failures can leave a returned tool outcome awaiting terminal persistence while its reservation excludes expired-lease recovery. Action timeout does not end this settlement context; success, a nonretryable error, or daemon cancellation is needed to release it. This is an introduced failure-containment tradeoff, not a verified authorization bypass.
Security review details

Security Blast Radius

  • inferred — The identified exposure is availability of Loop runs whose terminal persistence repeatedly fails in the daemon using this repository. Reservation state is shared within that repository instance; the evidence does not establish cross-service exposure or an unprivileged way to create reservations.

Trust Boundaries and Controls

  • observed — The new reservation path delegates to store enforcement instead of bypassing it. Reservation, completion, and failure retain the same run identity and claim token, and terminal mutations recheck token and settlement-actor authority.

Resilience and Maintainability Implications

  • observed — Successful reservation is released through deferred cleanup, and its callback is idempotent through sync.OnceFunc. Duplicate reservations are rejected. Daemon shutdown cancels the parent context, providing a bounded cancellation-grace escape from repeated settlement failures.

Hardening Proposals

  • proposed — If settlement needs a finite operational lifetime, define a durable outcome handoff and recovery state before ending its reservation. Merely timing out and releasing ownership could restore the duplicate-side-effect behavior this change is intended to prevent.

RetriggerConfidence Score: 5/5

[High risk] Adds database schema migration and retry logic for task persistence.

The PR appears safe to merge; no new actionable finding remains.

Summary

The PR reduces idle SQLite writer pressure and retains Loop action outcomes while terminal writes are retried. It also adds lease-settlement reservations, backs off failed scheduled claims, and verifies the migration and contention paths.

  • Reconciliation discovers candidates before taking the writer and repairs provenance after readiness.
  • Action result, heartbeat, and tool-event persistence gain bounded attempts; pending settlements are excluded from automatic lease recovery.
  • Migration 00126 adds reconciliation indexes without changing stored rows.

Move the new subtest after t.Parallel() on the parent test.

The new t.Run is placed before the parent's t.Parallel() call at Line 656. The parent test therefore runs the subtest while it is still serial. The subtest calls t.Parallel(), so it pauses until the parent function returns. Other subtests are registered after t.Parallel(). This ordering is inconsistent and changes the parent's parallel semantics for that subtest.

Move the block after t.Parallel() to match the other subtests.

All three actionable review threads are addressed in 4cdd27d. The additional CodeRabbit heartbeat-test nitpick is also fixed: parent t.Parallel runs before its subtests.

Real-SQLite regression coverage now includes shutdown grace, settlement beyond the lease deadline, recovery already waiting on the writer, reservation release, and deterministic provenance. The pre-commit checks passed. Further delivery validation uses the current-head CI without additional heavy local suites.

Current-head review disposition (4cdd27d88a6fb5407461f56efbca645a6e7b6bc3):

  • CodeRabbit approved with no actionable comments; its provenance finding and heartbeat-test ordering nitpick are fixed.
  • Greptile reports confidence 5/5 and no remaining actionable finding; both terminal-persistence P1 findings are fixed.
  • All three inline review threads are resolved.
  • The architecture note about retaining a completed result during repeated transient write failures describes the intended behavior required by [Loops] Reconciliation cycle blocks SQLite writes for ~20 s; tool results that land inside it are lost #689. Releasing its reservation on an arbitrary total timeout would permit replay of side effects. Each attempt and backoff remain cancellable, and daemon shutdown starts bounded settlement grace. A durable outcome handoff is a separate design, not required for this repair.
  • The automatic docstring-percentage warning is advisory; no missing behavioral documentation was identified. Existing source-comment conventions and the updated QA/impact documentation are retained without adding boilerplate to meet a generic percentage.

CI remains in progress. Merge will wait for the current head to pass.
