Test the MCP connector in three AI apps, then finish its connect guide #357

What to build

docs/mcp/README.md tells people how to connect docs.plus to an AI app. Each untested section carries a PENDING-HOST-TEST marker. A person connects each AI app to production, follows its section word for word, and runs every tool once. Each step that does not match the app gets corrected.

Tool smoke run

In each AI app:

1. create_document makes a scratch pad with two ## headings.
2. Open one heading's chat in docs.plus once.
3. find_documents finds the pad by title.
4. get_outline lists both headings.
5. read_document reads one section.
6. append_to_document adds a line at the end.
7. edit_blocks inserts one paragraph after block 2 of a section, and replace_text changes one word in that section. The heading stays, and no media is removed.
8. In Claude Code, read_document with a section_id returns the section text, not only metadata (see "Return the document and chat text where Claude Code reads it").
9. list_chat_rooms lists the open chat.
10. read_chat_thread reads it.
11. post_chat_message posts in it. The post shows in that chat in docs.plus.

Then follow §Disconnect. The app leaves Apps with access.

Acceptance criteria

• claude.ai: the steps work as written, and the smoke run passes. The consent page says Returns you to claude.ai.
• ChatGPT: the same, with Returns you to chatgpt.com.
• Claude Code: the same. The consent page shows http://localhost:<port> as an app on this computer.
• Each step that did not match is corrected in docs/mcp/README.md, with the app's current label text.
• The marker is gone from each section whose app passed. A failed section keeps its marker, and the failure is posted here.
• The Cursor, VS Code and Codex marker goes only if all three were tested.
• apps/hocuspocus.server/API.md §Supported hosts states the Claude Code result.
• If Claude Code fails, reopen RFC: should the MCP connector reach Claude Code as well? #231 with the failure.
• After the change is on main, the maintainer presses Refresh at https://context7.com/docs-plus/docs.plus.

Agent brief

Type: HITL. A person with claude.ai, ChatGPT with Developer mode, and Claude Code runs the tests. An agent then makes the doc edits from the posted results.

Verify: grep -n "PENDING-HOST-TEST" docs/mcp/README.md lists only untested or failed sections. bunx prettier --check docs/mcp/README.md passes.

Out of scope

• The Anthropic directory listing: RFC: should docs.plus submit its connector to the Anthropic directory? #232.
• Code changes. File a new issue for any tool that fails.
