Replaces the N-call function-impact ranking workflow: a single call
returns every unobserved db/http/remote boundary across all routes in
the target directory, ranked by route_count (how many distinct routes
have that boundary in their unobserved call tree).

- rank_unobserved_boundaries: pure function grouping UnobservedCallee
  entries by (name, file), deduplicating by route, sorting by
  route_count desc with name-ascending tiebreak.
- CriticalUnobservedEntry / UnobservedRouteRef: JSON output shape.
- `--role db|http|remote|all`, `--limit N`, `--json` flags.
- Human renderer: coloured ranked table with route exposure list.
- 7 new regression tests: ranking, dedup, role filter, statement_kind,
  tiebreak, route sort stability, empty-routes path.
