    * fix(opencode): follow debug mode for the server log level

A silent provider stall produces no error, so at the hardcoded --log-level
ERROR the diagnostic buffer is empty and there is no evidence the provider
request was even issued. A debug run now raises opencode to INFO, which
surfaces session/llm.ts's `stream` line (providerID/modelID/session/agent).
Measured: 0 INFO lines by default, 40 with RUNNER_DEBUG=1.

INFO and not DEBUG deliberately — `stream` carries only metadata, DEBUG can
carry payloads, and Actions logs are readable by anyone with repo read.

isMonitorDebugEnabled is exported as isDebugEnabled so one debug switch raises
every diagnostic we own, not just the activity monitor.

* fix(opencode): collapse the duplicate debug predicate, tidy the docs

Review round on #1148:
- log.ts:47 already held a private isDebugEnabled over the identical env triple,
  so promoting activity.ts's left two same-named predicates in one package.
  Collapsed onto activity.ts (imports only node:perf_hooks, so it is the acyclic
  home). Verified the reviewer's suggestion only partway: isRunnerDebugEnabled /
  isLocalDebugEnabled are NOT redundant — log.debug uses them to pick an output
  channel (::debug:: annotation vs a plain info line) — so those stay, with a
  comment saying why.
- JSDoc to lowercase per repo convention; LOG_LEVEL=debug spelled precisely.
- wiki lead-in still said 'two things block a useful debug run, both cheap to
  fix' after the first bullet became the instruction rather than a blocker.

Re-verified after the refactor: banner + stream line still present under
RUNNER_DEBUG=1, run completes successfully.
