Complexity refactors: 61 functions under the thresholds #312
Description
Tracking issue for docs/plans/complexity-refactors.md — the 4b-3 ladder's chartered remainder: 52 gocognit (threshold 20) + 9 gocyclo (threshold 15) findings, refactored by behavior-preserving decomposition across seven leaf-to-core phases, one PR each. Discipline and phase table in the plan doc.

Complete. All seven phases merged (#313–#319): 61 planned findings plus the surfaced plan-table omission (lint.Go 31) decomposed behavior-preserving under gocognit 20 / gocyclo 15. The repository's full lint output — every linter, uncapped — is now empty; the unmodified test suite passes throughout. Plan: docs/plans/complexity-refactors.md (status: complete, per-phase notes recorded).
