Followup on #569
Filed as a separate issue as #569 has already grown into a beast. The PR records two backend-specific deficiencies that the dialect system does not take care of and may require some considerable refactoring:
MySQL: Can't reference a temp table multiple times in the same statement
MS SQL: Nested ORDER BY not allowed
These are documented in the live tests with examples
